An impending clinical trial by the NHS, named “Pathways,” which plans to test puberty blockers on young individuals, has sparked controversy and significant backlash. ADF International, a legal advocacy organization, has issued strong opposition, urging the NHS to halt what they describe as “reckless experimentation on children.”
Set to involve approximately 220 participants aged 10 to 16, the trial has raised alarms due to the potential “lifelong and often irreversible consequences” identified by ADF International. The British government had previously prohibited the prescription of such medication to minors in 2025, following significant findings from the Cass Review.
This independent review highlighted the substantial risks associated with puberty blockers, particularly for minors. The report underscored the largely unknown long-term effects on cognitive development, psychological growth, and physical health. Known side effects include impacts on fertility, sexual function, and bone density, with puberty blockers often being the precursor to cross-sex hormones and surgical procedures.
ADF International criticized the ability of minors to give informed consent regarding these drugs, stating, “Puberty blockers are the first step down a medical path with extremely dangerous lifelong consequences.” The organization also questioned the scientific merit of the trial, emphasizing that a two-year study cannot adequately assess lifelong impacts on identity, fertility, or overall well-being.
“Calling this a ‘clinical trial’ doesn’t make it safe,” said ADF. “It’s exposing society’s most vulnerable to the lie of gender ideology and its incredibly harmful effects. Too many children already have been harmed because ideology has overtaken evidence. This trial perpetuates these same grave harms.”
The future of the trial remains uncertain as legal challenges arise. A judicial review brought forth by detransitioner Keira Bell and therapist James Esses argues against the legality of such experimentation on children by the NHS, especially in light of its own review pointing to severe long-term risks.
